Then the details of the person who is getting the guardianship rights should be added including full name, contact information, phone number and additional details if required. The next thing to add here is the details of the kid(s) who will go into the legal custody of the guardian including their names, genders, and ages.
Reason(s) for Writing the Guardianship Form:
The next part of the guardianship form includes the reasons for allowing legal custody either full or partial to someone. For instance, you are a soldier and you are going overseas for duty and there is no one back at home to take care of your kids. So, you can allow someone i.e. your friend or a relative to look after your kids in your absence and this reason should be mentioned in the guardianship form clearly. Other reasons might include going to jail, getting divorced, going for medical treatment etc.
Type of Guardianship Form:
The type of the guardianship should be mentioned in the form as this portion defines the legal rights, responsibilities, and obligations of the guardian. For instance, if you are only allowing partial guardianship to the other party, you should mention that clearly here and if this guardianship is with full rights and responsibilities, it should be specified here.
Duration of the Guardianship Form:
Whether we are talking about partial or full guardianship, there is a specific duration for which the agreement is valid for i.e. 6 months, 1 year or 5 years. Usually after this duration is over, it’s time to either cancel the guardianship or renew it for another couple of years.
